JOB SUMMARY

The Director Case Management is responsible and accountable for the implementation of the case management program across the hospital system. The components/roles of the program consist of the following: care facilitation, continuity of care, case management, and discharge planning. The Director is responsible for developing systems and processes for care management and planning at the system level. In addition, the Director evaluates and ensures that hospital resources are used appropriately and effectively and that established outcome measures are met. The Director promotes interdisciplinary collaboration, fosters teamwork, and champions service excellence. Above all, this position serves to exceed the expectations of patients and all other customers we serve.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

– Other duties may be assigned.

· Plans, directs, and supervises all aspects of the case management program.

· Oversees a team who utilizes professional skills to assess patient and family scenarios for clinical appropriateness for the setting and establishes plans for effective management of the continuum of care.

· Facilitates growth and development of the program consistent with the organization mission and in response to the dynamic nature of the health care environment through best practices, networking, and quality management.

· Responsible for identifying and achieving optimal targeted financial, quality, and related organizational outcomes.

· Ensures staff compliance with organizational policies and external regulatory agencies and takes leadership responsibility to coordinate the integration of the department's patient care and discharge planning processes with related hospital departments and external agencies to ensure continuity of care and optimal efficiency of care.

· Ensures continuity of care for patients and coordination of discharge planning needs.

· Responsible for analysis of length of stay and readmission rates and meeting goals related to length of stay and readmission. Ensures team participation in multidisciplinary rounds and complex case reviews.

· Acts as a liaison to facilitate communication and collaboration between all care partners (physicians, hospitalists, community agencies, nurses, utilization review team, etc.).

· Uses data to drive decisions, plan, and implement performance improvement strategies to meet targets and organizational goals.

· Educates peers and multidisciplinary team members on case management functions and processes.

· Ensures safe care to patients by adhering to policies, procedures, and standards, within budgetary specifications, including time management, supply management, productivity, and accuracy of practice.

· Demonstrates proficiency in use of clinical decision support systems, data analysis, and data reporting to transform data into information to support the outcomes management process.

· Demonstrates leadership by guiding organizational and individual growth through creation and communication of a compelling vision and strategy.

· Demonstrates the ability to develop a strategic vision for department operations and translate it into specific measurable plans and tactics.

· Ability to effectively plan and implement departmental and organizational change.

· Sets and maintains the highest levels of service to patients, family members, visitors, physicians, and other hospital departments.

· Utilizes performance management expertise to facilitate the achievement of department goals.

· Holds staff accountable for adherence to department and hospital policies and procedures.

· Demonstrates the highest level of knowledge of the specific arena of the healthcare environment that supports the mission of the NCH HealthCare System.

· Demonstrates effective communication and creates an environment of collaboration and partnership to ensure effective working relationships.

· Demonstrates ability to develop effective alliances within the organization and the system.

· Demonstrates ability in financial planning and analysis for the department. Allocates resources to effectively staff department and meet productivity and quality goals.

· Ability to analyze operational problems and develop creative solutions using sound business management principles.

· Oversees various personnel actions including, but not limited to, hiring, performance appraisals, disciplinary,

actions, terminations, and other related activities.

· Understands and accepts the continual nature of change associated with the healthcare environment and the

importance of remaining flexible to meet changing demands and assist staff to understand change.

    EDUCATION, EXPERIENCE AND QUALIFICATIONS

    · Minimum of a BSN required.

    · Master’s degree in health-related field preferred.

    · Minimum of 3 years leadership experience.

    · Case management or leadership Specialty professional certification strongly desired.

    · Licensed as a Registered Nurse (RN) in the State of Florida.

    · Knowledge of Joint Commission and CMS case management regulatory requirements.
