JOB SUMMARY

The Clinical Specialist Pharmacy Oncology is responsible for the provision of safe, effective, evidence-based Hematology/Oncology pharmacotherapy services to patients. Pharmacotherapy services provided include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into chemotherapy-based regimens or oncology/hematology pharmacotherapy, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Proficiently provides direct patient-centered care and integrated pharmacy services in practice setting with physicians, nurses, and other hospital and Outpatient Infusion Services (OPIS) personnel. Are aligned with the target interdisciplinary programs and services to deliver and integrate oncological medication therapy management throughout the institution. Serves as departmental resource, and liaison to other departments, hospital personnel, OPIS, or external groups. Provides medication and practice-related education/training to staff members and actively serves as preceptor for Doctor of Pharmacy (Pharm.D.) students and pharmacy residents. Participates in clinical pharmacy committees. This position reports to the System Pharmacy Clinical Manager. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

– Other duties may be assigned.

· Provides direct Oncology pharmacotherapy services and pharmaceutical patient care to assure and promote drug utilization activities that are aligned with patient care needs, evidence-based practices, and regulatory standards.

    · Maintains proficiency in contemporary pharmacy services and clinical pharmacy programs.

    · Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized oncology patient care areas and Outpatient Infusion Services (OPIS) to provide oncological patient-centered care.

    · Identifies high-risk patients and implements interventions to improve quality and safety.

    · Makes appropriate evidence-based, patient-centered medication recommendations.

    · Participates in the management of medical emergencies.

    · Provides accurate safe, timely medication review, reconciliation, and counseling as appropriate.

    · Maintains proficiency in decentralized oncology pharmacy services and oncology clinical pharmacy programs.

    · Maintains proficiency in hospital computer systems and medication ordering systems.

    · Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and assures continuity of pharmacotherapy care during transitions of care.

    · Actively participates in stewardship activities and restricted medication programs.

      · Facilitates medication procurement, ordering, and dispensing procedures.

      · Oversees pharmacy operations and technician practice/activities as defined by state laws.

      · Participates in Clinical Pharmacy Committees of the NCH Pharmacy Department and Pharmacy & Therapeutics Committee.

        · Identifies, designs, implements, and tracks improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs, routinely benchmarks against local and national best practices.

        · Actively identifies practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice.

          · Assist in developing and maintaining the hospital formulary, guidelines and use evaluations, clinical pathways, and order sets.

            · Assures pharmacy is an integral part of the health-care delivery system and facilitates enhancement and expansion of pharmacy services/programs.

              · Participates in policy and ordersets development/review

              · Facilitates experiential education and practice advancement of pharmacy residents and pharmacy students and other staff members.

                · Maintains proficiency in preceptor roles and actively emulates education/mentorship skills.

                · Develops training experiences/competencies and creates new relationships for teaching and training opportunities.

                · Provides education/competencies to pharmacists, providers, nurses, and other healthcare staff on protocols, guidelines, and department initiatives as appropriate.

                · Understands and applies principles and concepts of Performance Improvement.

                  · Looks for opportunities and solutions for process improvements and seeks to improve job skills.

                  · Serves as a role model, demonstrates creativity, cooperation, innovation, peer support.

                  · Consistently contributes to department goals; Takes active role in training new staff, is proactive in working toward department goals.

                  · Participates in quality management programs, pharmacy committees, and interdisciplinary committees with the goal of improving services by monitoring processes, analyzing data, implementing interventions to improve, evaluating the effectiveness of those interventions, and providing guidance and education to staff on departmental and institutional management priorities, projects, or initiatives.

                    EDUCATION, EXPERIENCE AND QUALIFICATIONS

                    · Bachelors of Science from College of Pharmacy Accredited Required. Doctor of Pharmacy degree (Pharm.D) preferred.

                      · State of Florida Pharmacist license.

                      · Completion of ASHP-accredited Pharmacy Residency program in hospital environment OR 2 years of Clinical hospital pharmacy experience

                      · Board of pharmacy specialty certification(s) preferred.

                      · Advanced Life Support (ALS) Certification – Must be obtained within 6 months of hire.

                      · Effective oral and written communication skills, as well as demonstrated interpersonal and team skills are essential.

                      · Knowledge of contemporary hospital/clinical practice and service is essential.

                      · Demonstrated ability to work independently with minimal direct supervision, with the ability to set priorities and make critical decisions.

                      · Epic software experience preferred.

                      · Intermediate computer knowledge required: Uses Microsoft Word, Excel, Outlook, PowerPoint, Teams, and Windows
