JOB SUMMARY

The Clinical Specialist Pharmacy – Emergency Medicine is responsible for establishing and expanding clinical services in the emergency departments across the healthcare system. This includes the development and maintenance of medication clinical programs, protocols, order sets, medication utilization reviews and performance improvement projects to optimize safe and effective pharmacotherapy in the Emergency Department. Provides mentorship and is a clinical resource for staff and other health care providers. Serves as departmental resource, representative and/or liaison to other departments, hospital personnel, or external groups/committees. Provides expertise for clinical activities relating to various programs for the department of pharmacy to ensure optimal patient outcomes. Actively serves as preceptor for Doctor of Pharmacy (Pharm.D.) students and pharmacy resident and leads clinical pharmacy subcommittee(s). This position functions across the healthcare system’s Emergency Departments and reports to the System Clinical Pharmacy Manager.

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Responsible for oversight of pharmacy clinical services and programs across the hospital-based Emergency Departments and Free-Standing Emergency Departments (FSEDs) within the healthcare system to promote safe and effective medication utilization activities that are aligned with patient care needs, evidence-based practices, and regulatory standards.

· Establishes and maintains oversight of the medication admission reconciliation program.

· Establishes and maintains culture call back program.

· Works as an active member of multidisciplinary team and collaborates with healthcare providers to provide patient-centered care.

· Identifies high-risk patients and implements interventions to improve quality and safety.

· Participates in the management of medical emergencies.

· Promotes and educates on stewardship activities and restricted medication programs.

· Facilitates medication procurement, ordering, and dispensing procedures.

· Oversee pharmacy operations and technician practice/activities as defined by state laws.

· Identifies, designs, implements, and tracks improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs; routinely benchmarks against local and national best practices.

· Assists in developing and maintaining the hospital formulary, guidelines and use evaluations.

· Facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice.

· Develops and maintains department policies, protocols, and order sets to optimize drug therapy.

· Assist in developing collaborative practice agreements for pharmacy service lines.

· Maintains proficiency in hospital computer systems and medication ordering systems.

· Provides clinical and distributive pharmacist functions as assigned.

· Facilitates experiential education and practice advancement of pharmacy residents and pharmacy students and other staff members.

· Maintains proficiency in preceptor roles and actively emulates education/mentorship skills.

· Develops training experiences and creates new relationships for teaching and training opportunities.

· Provides education/competencies to pharmacists, providers, nurses, and other healthcare staff surrounding protocols, guidelines, and department initiatives as appropriate.

· Leads and/or participates as needed in quality management programs, pharmacy subcommittee(s), and interdisciplinary emergency medicine committees with the goal of improving pharmacy services by monitoring processes, analyzing data, implementing interventions to improve, and evaluating the effectiveness of those interventions.

· Understands and applies principles and concepts of Performance Improvement.

· Looks for opportunities and solutions for process improvements and seeks to improve job skills.

· Serves as a role model, demonstrates creativity, cooperation, innovation, peer support.

    · Consistently contributes to department goals; Takes active role in training new staff, is proactive in working toward department goals.

      EDUCATION, EXPERIENCE AND QUALIFICATIONS

      · Doctor of Pharmacy degree (PharmD) required.

      · State of Florida Pharmacist license required.

      · PGY1 residency with 2 years practice experience in area of specialty and board certification (BPS) OR PGY-2 Pharmacy Residency (must have BPS within 1 year of hire) OR if no residency training, must have a minimum of 3 years of practice experience in area of specialty (must have BPS within 1 year of hire).

      · Board of pharmacy specialty certification(s)

      · Epic software experience preferred.

      · Advanced Life Support (ALS) Certification – Must be obtained within 6 months of hire.

      · Advanced Pediatric Life Support (PALS) Certification – Must be obtained within 6 months of hire.

      · Knowledge of contemporary hospital/clinical practice and service is essential.

      · Demonstrated ability to work independently with minimal direct supervision, with the ability to set priorities and make critical decisions.

      · Effective oral and written communication skills, as well as demonstrated interpersonal and team skills are essential.

      · Intermediate computer knowledge required: Uses Microsoft Word, Excel, Outlook, PowerPoint, Teams, and Windows
