JOB SUMMARY
The RN Navigator Total Joint acts as a resource to patients/families, nursing staff, physicians, allied health professionals, and community members in the care of patients undergoing joint replacement surgery. The Navigator ensures that services provided are in alignment with current clinical practice guidelines throughout the continuum of care. Program activities are planned, developed, implemented and monitored in close collaboration with the Total Joint Replacement Interdisciplinary Team, System Total Joint Coordinator and Nursing Medical/Surgical Director. This position coordinates preparation for patient care transitions from office setting to surgery, recovery during hospitalization and after discharge process while keeping the patient as the primary focus.
ESSENTIAL DUTIES AND RESPONSIBILITIES

· Shares knowledge and expertise regarding evidenced based practices in the care of total joint patients.
· Identifies the most current evidence based clinical practice guidelines (CPG’s) related to total joint and programs which help to achieve and maintain the program’s mission, vision, goals, and objectives.
· Collaborates with the Total Joint System Coordinator to develop and implement policies, standards and programs which help to achieve and maintain the program’s mission, vision, goals and objectives.
· Assists with Pre-op Education to patients and families planning surgery and provide continued guidance throughout recovery and post hospitalization.
· Assists with tracking the progress of the program and identifying areas that require improvement.
· Performs concurrent and retrospective chart reviews and data abstraction. Maintains the total joint database in a timely manner.
· Promotes patient, family and physician satisfaction and is accountable for patient care delivery through appropriate resource utilization, active communication/follow up with team members and promoting clinical quality initiatives.
· Assists in the management and coordination of THR and TKR care throughout the continuum of care.
· Works autonomously, possesses excellent problem solving skills and is able to complete assigned duties with timeframe given.
· Maintains clinical knowledge and skill for the total joint patient population. Show a commitment to ongoing personal and professional growth through internal and external learning opportunities.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
· Current Florida RN Nursing License.
· Bachelor’s Degree in Nursing with 4 years of clinical Orthopedic experience or Associate’s in Nursing with a minimum of 5 years of clinical Orthopedic experience required.
· Orthopedic nurse certification preferred.
· Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
· Experience with Adult Learning Principles including actual teaching experience preferred.
· Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
