JOB SUMMARY
The RN Diabetes Educator provides patient, family and staff education on the management of diabetes and coordinates care for a group of patients (high risk, high cost, complex) and tracks progress toward improved/cost effective care. Serves as a patient advocate.
ESSENTIAL DUTIES AND RESPONSIBILITIES

· Provides education to patients and families as appropriate on diabetes self-management and survival skills to promote compliance with medical regimen and adaptation to a sudden life changing condition.
· Performs patient needs and resource assessment and collaborates with nursing and social services staff to coordinate discharge plans that ensure services and resources are received.
· Maintains flexible work patterns to meet needs of staff/patients.
· Communicates with health team members, department staff and other ancillary departments to assist in determining best practice and ensure efficient and accurate communication.
· Develops and provides diabetes education programs and equipment training/testing to nursing and support staffs. Acts as a resource for diabetes management/care to nursing and support staff.
· Uses age appropriate learning methods, appropriate behavioral/motivational strategies and reinforcement techniques.
· Creates an educational setting conducive to learning with adequate resources to facilitate the learning process.
· Demonstrates understanding of the stages of behavior change and ability to counsel individuals on these stages through development of behavior change goals.
· Assesses patient readiness to learn and patient perceived needs.
· Facilitates educational/behavioral experiences that assist the patient in achieving identified goals.
· Participates in focused studies to resolve patterns in aggregate variances. Participates in development and updating of protocols to enhance outcomes.
· Promotes NCHHCS by providing/supporting diabetes education/awareness programs in the community.
· Assumes responsibility for professional development and continuing education to acquire current knowledge and skills.
· Understanding of and shares responsibility for the fiscal efficiency of the diabetes education and nutrition programs.
· Accepts responsibility for self-assessment of performance and peer review to assure the delivery of high-quality diabetes education.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
· Licensed as a Registered Nurse in the state of Florida and a minimum of one year of experience in the care of people with diabetes.
· Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
· Certified Diabetes Educator (CDE) preferred, or actively pursuing.
· Bachelor of Science in Nursing is preferred.
· Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
