JOB SUMMARY
The Registered Nurse Case Manager is an employee of NCH Healthcare System with a dotted line to the VP of Recovery Programs at St. Matthew’s House for daily supervision. This role will interact with Neighborhood Health Clinic and NCH Physician leadership team. This role is responsible for the overall assessment of St. Matthew’s House resident medical issues and securing appropriate resources to address resident needs. This role identities the level of care needed and develops a treatment plan(s) in conjunction with Neighborhood Health Clinic Medical and Dental Volunteers to improve the overall health of St. Matthew’s House residents.
The RN Case Manager shall ensure the care and treatment plans adhere to St. Matthew’s House mission statement and goals. The RN Case Manager will perform this work at one of several St. Matthew’s House locations. St. Matthew’s House is a faith based 501(c)(3) charitable organization that exists to transform lives in a spiritual environment that is both compassionate and disciplined.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Other duties may be assigned.
· Provides medical care to residents within the scope of an RN license including:
· Nursing triage.
· First aid and wound care.
· Administrating and monitoring medication protocol.
· Determining urgent and/or emergency situations and taking appropriate action.
· Maintains medical equipment and supplies.
· Conducts assessment of resident’s general health and provides practical advice to improve overall physical health.
· Serves as a scheduling liaison between S. Matthew’s House, NCH Healthcare System, Neighborhood Health Clinic, and other healthcare partners.
· Trains St. Matthew’s House Shelter and Program Staff in CPR & First Aid.
· Participates in outreach to the chronic homeless in our community.
· Documents pertinent information in the client management system (Mission Tracker).
· Seeks appropriate guidance from Neighborhood Health Clinic and SMH leaders and works closely with SMH staff.
· Communicates regularly with Neighborhood Health Clinic and SMH VP of Recovery Programs to ensure a smooth launch of the program and continuous quality of care is administered for the life of the program.
· Completes all necessary medical and administrative documentation for NCH, NHC, and SMH.
· Models desired behaviors to residents through personal example.
· Maintains confidentiality of resident information and maintains compliance with all HIPAA regulations.
· Engages in development opportunities to maintain Florida Registered Nurse License.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
· Bachelor’s degree in Nursing preferred.
· Minimum of 2 years of experience in Nursing.
· Licensed as a Registered Nurse (RN) in the state of Florida.
· Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
· Experience with patient assessment and health histories required.
· Knowledgeable regarding contagious illness and treatment to protect.
· Must be well organized and detail oriented.
· Must possess and demonstrate critical thinking skills.
· Must be able to remain calm under pressure and successfully manage multiple demands.
· Solid communication and collaboration skills required.
· Must be able to demonstrate concern for others’ well-being.
· Must be a cultural fit, team player, abide by SMH Core Values, and have a heart to help those SMH serves.
· Must have a demonstrated record of working with highly confidential information and knowledgeable of HIPAA laws and compliance.
· Must be able to stand, sit, bend, and lift at least 10 pounds.
· Must be able to work in an office environment with required COVID-19 precautions.
· Must be able to travel occasionally to various SMH locations.
· Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
