JOB SUMMARY
The Clinical Manager for Population Health and the NCH Medical Group Accountable Care Organization (ACO) is responsible to act as a change agent, incorporating the roles of clinical specialist, educator, researcher, and manager as indicated to support and enhance patient care through initiatives that drive value and improve health outcomes for specific populations of patients with chronic conditions who use our primary care and specialty services and are attributed to one or more value based programs the NCH Medical Group participates in. Supports the NCH Medical Group practices and leadership in the development and implementation of population health and value-based strategies and initiatives. Provide leadership and supervision for Population Health/ACO team. Collaborates with leadership across the organization to align strategy, understand the practices, and drive population health initiatives forward.
ESSENTIAL DUTIES AND RESPONSIBILITIES
– Other duties may be assigned.
· Work in partnership with the Administrator of Analytics, Quality, Compliance, Patient Access and Special Projects to advance the clinical initiatives of the Population Health team, including but not limited to improved complex care collaboration with internal and external entities, the development and implementation of chronic disease registries, and development and implementation of acute and chronic disease care pathways/institutional guidelines.
· Measure, analyze, and evaluate care quality and financial performance, respective to NCH Medical Group value-based contracts.
· Provide clinical and operational guidance to Population Health team through understanding current capabilities, organizational initiatives, and necessary areas for improvement.
· Collaborate with care delivery teams throughout the organization to drive population health initiatives forward.
· Serve as a conduit and educator to disseminate population health updates, data insights, and recommendations to providers and their support staff throughout the Medical Group and hospital-based teams.
· Work alongside the Administrator of Analytics, Quality, Compliance, Patient Access and Special Projects and the NCH Medical Group Operations Leadership to build strong and successful population health clinical strategies that will increase quality and financial incentives.
· Provide clinical expertise for population health technology solutions.
· Performs and participates in other projects as assigned and as the needs of the population health team evolve.
· Uses adult education principles and instructional design to provide an effective and efficient system of staff education.
· Designs and implements a system for assuring initial and ongoing competency of staff.
· Provides population health focused education to external agencies, community, and other health professionals.
· Provides leadership for and participates in practices, divisional, departmental, and/or interdepartmental committees.
· Serves as change agent in the NCH Medical Group practices, and hospital settings when applicable.
· Assumes responsibility for administrative activities and special projects as delegated by the Administrator of Analytics, Quality, Compliance, Patient Access and Special Projects, the NCH Medical Group Operational Leaders.
· Contributes towards the achievement of departmental goals as delegated.
· Serves in a consultative and resource capacity to other divisions, departments, and the community as needed.
· Participates in professional clinical activities in the department, hospital, and community.
· Participate in direct patient care coordination in support of ACO operations and population health initiatives.
· Provide direct supervision for the Population Health team members.
· Incorporates the principles of Service Leadership to ensure positive patient/customer relations.
· Help develop a multi-disciplinary complex care team.
· Adheres to the NCH Medical Group policies regarding personal conduct, following established leadership fundamentals, remaining responsive to instruction and coaching from Administrators as well as meeting the standards of personal appearance, attendance, and punctuality.
· Partners with other organizational teams such as the Center for Innovation and Learning to identify mechanisms to participate in continued development or expansion of programs.
· Oversee the education of patients on disease management, symptom recognition, and prevention strategies.
· Oversee the distribution and collection of clinical attestations provided by the insurance companies.
· Oversee communicate with the NCH Medical Group offices on patient status via the use of our EHR, site visits, and telephonically.
· Oversee the population health team on correct document communications with the patients, the offices, and the providers using EPIC.
· Oversee population staff when monitoring patients and helping them to execute on their care plans, health goals, their adherence to treatment plans, medications, and health outcomes.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
·Minimum of Associates Degree required, Bachelors degree preferred.
· Minimum of 5 years of applied clinical experience as an LPN, higher degree preferred.
· Licensed as an LPN or higher degree in the State of Florida.
· Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.
· National certification by area of specialty preferred.
· Excellent organizations, interpersonal, oral, and writing communication skills required.
· Exemplary leadership qualities.
· The ability to relate and deal effectively with physicians, administrators, support staff, and the general public with a high degree of tact and discretion is required.
· Must have working knowledge and demonstrated experience in leading population health initiatives for both the ACO and other value-based initiatives.
· Must be able to prioritize and work independently.
· Good customer services and communication skills.
· Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
