JOB SUMMARY
ESSENTIAL DUTIES AND RESPONSIBILITIES
• Maintains proficiency in pharmacy services and clinical pharmacy programs.
• Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered care.
• Identifies high-risk patients and implements interventions to improve quality and safety.
• Makes appropriate evidence-based, patient-centered medication recommendations.
• Participates in the management of medical emergencies.
• Provides accurate safe, timely medication review, reconciliation, and counseling as appropriate.
• Maintains proficiency in decentralized pharmacy services and clinical pharmacy programs.
• Maintains proficiency in hospital computer systems and medication ordering systems.
• Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and assures continuity of pharmacotherapy care between shifts and among staff.
• Actively participates in stewardship activities and restricted medication programs.
• Facilitates medication procurement, ordering, and dispensing procedures.
• Oversees pharmacy operations and technician practice/activities as defined by state laws.
· Chairs subcommittee(s) of the NCH Pharmacy Department and Pharmacy & Therapeutics Committee
• Identifies, designs, implements, and tracks improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs, routinely benchmarks against local and national best practices.
• Assist in developing and maintaining the hospital formulary, guidelines and use evaluations, clinical pathways, and order sets.
• Participates in policy and ordersets development/review
• Maintains proficiency in preceptor roles and actively emulates education/mentorship skills.
• Develops training experiences/competencies and creates new relationships for teaching and training opportunities.
• Provides education/competencies to pharmacists, providers, nurses, and other healthcare staff on protocols, guidelines, and department initiatives as appropriate.
· Understands and applies principles and concepts of Performance Improvement.
• Looks for opportunities and solutions for process improvements and seeks to improve job skills.
• Serves as a role model, demonstrates creativity, cooperation, innovation, peer support.
• Consistently contributes to department goals; Takes active role in training new staff, is proactive in working toward department goals.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
· State of Florida Pharmacist license.
· Completion of ASHP-accredited Pharmacy Residency program in hospital environment OR 2 years of Clinical hospital pharmacy experience
· Advanced Life Support (ALS) Certification – Must be obtained within 6 months of hire.
· Intermediate computer knowledge required: Uses Microsoft Word, Excel, Outlook, PowerPoint, Teams, and Windows
· Effective oral and written communication skills, as well as demonstrated interpersonal and team skills are essential.
· Knowledge of contemporary hospital/clinical practice and service is essential.
· Demonstrated ability to work independently with minimal direct supervision, with the ability to set priorities and make critical decisions.
Additional Preferred Qualifications:
· Doctor of Pharmacy degree (Pharm.D)
· Board of pharmacy specialty certification(s)
· Epic software experience
