JOB SUMMARY

The Clinical Coordinator Donor Relations will develop and maintain substantive relationships with current and prospective Medical Diplomats/donors and NCH friends in the community. The Clinical Coordinator for Donor Relations will serve as a direct link to hospital staff, Medical Staff, and appropriate outside organizations. The concierge services provided by the Clinical Coordinator Donor Relations will be coordinated with specific nursing unit functions and projects. This coordination should maintain the effectiveness of these programs and reinforce world-class service and patient satisfaction. Responsible for coordinating care across providers and departments to ensure timely, efficient, and patient-centered service delivery. Access to PHI is required to support clinical communication, care planning, and continuity of care. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Executes superior customer service to donor-patients of the Medical Diplomats Council program. Ensures patient satisfaction and fulfillment of needs on an individual basis. Provides guidance and consultation, adapts, and implements improved operational processes on a case-by-case basis. Use personal judgment and initiative to develop effective and constructive solutions to accomplish Patients’ goals and needs.

· Works with the Director to continuously educate Hospital Staff in Emergency Departments, Admitting, Nursing and other related areas, regarding the functionality of the Medical Diplomats Council and reassure a non-two-tier system. Actively cultivates a strong personal working relationship with the NCH Physician group, other physicians, and hospital staff as well as area Concierge medical practices for the continuous success of the program.

· Acts as one of the primary, dedicated contacts for participants of Medical Diplomats Council by answering telephone calls, pagers, voice mail and correspondence. Provides a high level of service and attention (in person and via telephone, at hospital, at CFP, at donors' office or home, as requested and appropriate) on a case-by-case basis. Respond in person, whenever possible, to assist patients of Medical Diplomats Council when they present to the Emergency Department, Admitting or other area of NCH, as needed, to answer questions and help navigate the healthcare system.

· Navigates Medical Diplomats tactfully and diplomatically on a personalized, individualized basis through the NCH Healthcare System making the experience as comfortable and convenient as possible.

· Provides a wide variety of healthcare related information and service coordination as requested, including assistance with emergency room visits, hospital room assignments, pre-admitting, planned procedures, surgeries, physician referrals, appointment setting, urgent care coordination, medical record transfers, and coordination with other healthcare providers. Works closely with healthcare professionals to provide this assistance, both within NCH and other accredited organizations to provide high quality service to top donors.

· Manages and coordinates with CFP staff, personalized requests for Medical Diplomats such as tours, plaques, and meetings with executive management and/or medical staff.

· Conducts daily patient visits throughout the hospital; rounding on VIPs, donors, or friends of NCH, to develop, sustain and maintain relationships with NCH, and coordinates this activity with appropriate individuals of the NCH Healthcare System including nurse leadership and clinical care team.

· Handles patient information as allowed in compliance with all HIPAA standards.

· Responds and seeks to resolve issues identified by patients and family members

· Provide daily rounding and personalized patient/family contact. Assists patient/family as requested.

· Visits other patients throughout the hospital who have been identified as important VIPs, donors, or friends of NCH, as a way to develop, sustain and maintain a relationship to NCH, and coordinate this activity with the appropriate individuals of the NCH Healthcare System.

· Demonstrates effective professional communication and excellent customer service skills based on positive attitudes and values.

· Understands and effectively uses donor base (i.e. Raisers’ Edge) for donor information, retrieval, and reporting.

· Responsible for demographic data entry into donor database and EMRsystems. Administrative tasks include database entry, running and analyzing reports.

· Appropriately records interactions with Medical Diplomats, documents interactions appropriately

· Supervisory responsibility for Demographic Data Entry into donor database system.

· Management of Admitting records process, ensuring donor records are kept up-to-date and reflected within the NCH Admitting Department, including semi-annual audits.

· Responsibility for Medical Diplomat Council digital card processing, recognizing current Medical Diplomats.

· Assists with the planning, coordinating, and conducting Medical Diplomat Council interviews, one-on-one meetings with patients.

· Coordinates educational events for Diplomats such as seminars, physician lectures, and Town Halls.

· Coordinates stewardship events such as Medical Diplomats Holiday Dinner Party and other appreciation events.

· Timely record keeping of all interactions with Diplomats in the donor database system.

· Attends NCH CFP-sponsored and community affiliated events as required.

· Maintains up-to-date knowledge and expertise regarding NCH’s philanthropic needs.

· Demonstrates positive and effective interpersonal skills through verbal and nonverbal communication with patients, families, peers, supervisors, and physicians.

· Reviewing medical records and patient charts to assess care needs

· Communicating with healthcare providers regarding patient status, discharge planning, and follow ups

· Collaborate with patients and the care team or providers to accommodate any special needs the patient may have

· Serve as liaison between patient and care team as necessary

· Assist patient in coordinating care throughout NCH

· Accompany patients to medical appointments as needed/necessary

· Assist patient in connecting with supportive care services

· Provide navigation for patients by addressing any logistical barriers, scheduling complications, etc., that would prevent a patient from showing up at their appointment

· Ensure appropriate discharge information provided post-visit is understood by the patient

· Follow-up with patient’s post-visit

· Various administrative tasks as assigned.

    EDUCATION, EXPERIENCE AND QUALIFICATIONS

    · Minimum of Bachelor’s Degree with 3 years’ experience in clinical services or related field OR Associate Degree with 6 years’ experience in clinical services or related field OR High School/GED with 9 years’ experience in clinical services or related field required.

    · Minimum of 1 year of experience in having exercised tact and judgement when responding to situations delicate in nature or dealing with people in distress.

    · Clinical Certification required.

    · Good analytical and problem-solving skills are vital as well as the ability to understand, interpret and apply specific administrative policies.

    · The ability to analyze, synthesize and concisely communicate information is vital. 

      · Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
