JOB SUMMARY
The Clinical Specialist will provide good quality pharmaceutical care in a timely manner.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Other duties may be assigned.
· Work closely with nursing and physicians, and as always, to be a source for any questions or concerns that they may have.
· Assists the social workers and her assistant with the “Patient Assistance Program” and “Patient Drug Replacement Program”. Identify the quantity of medication needed and/or received for patient’s chemotherapy and supportive protocol.
· Demonstrates the skills and professional judgement required to provide pharmacokinetic dosing/monitoring consultation.
· Review and verify chemotherapy orders and flow sheets for accepted protocols/supportive therapy; accurate patient demographics; accurate dose calculations and medication administration information.
· Assist the Director of Pharmacy with implementing new or revised services, to improve patient care and satisfaction.
· Provide inventory control and routinely check medications for outdated product per pharmacy practice standards and JCAHO guidelines.
· Demonstrates the skills and professional judgement required to provide therapeutic drug monitoring, pharmacokinetic monitoring, and work directly with the medical staff in the development of care plans.
· Assists in adverse drug reaction monitoring and compliance with JCAHO standards.
· Demonstrates skills and judgement necessary to provide pharmaceutical care services.
· Monthly inspections are on time and accurate.
· Maintains the highest degree of accuracy as measured by lack of variances. Actively reviews patient medication profiles for demographic completeness, ADRs, drug interactions, therapeutics duplications, formulary compliance and therapeutic appropriateness.
· Good documentation of interventions.
· ADR’s and variances identified and written when appropriate.
· Technician Supervision.
· Participates in at least one: case presentation, community service; P&T presentation or project; DUE; Policy and Procedure development or training; technician/pharmacist training.
· Understands the principles and concepts of Performance Improvement. Looks for opportunities and solutions for process improvements and seeks to improve job skills.
· Understands the legal/regulatory requirements related to the release of information. Able to use the information systems pertinent to the job.
· Creative Problem Solving- Examples of Achievement.
· Appearance: Closed toe shows, appropriate hair length, ID Badge visible. RpH: Clean, pressed lab coat outside the department.
· Actively assists and supports departmental activities to ensure that operational and salary expenses are equal to or less than approved budget,
· Understands the principles and concepts of Performance Improvement. Looks for opportunities and solutions for process improvements and seeks to improve job skills.
· Safety: Infection Control Policies followed; JCAHO/Medicare/State/Dept. policies followed
· Pharmacist will be responsible for all facets of drug therapy in the Hematology/Oncology patient population within the NCH Healthcare System.
· Serves as a role model, demonstrates creativity, cooperation, innovation, peer support.
· Consistently contributes to department goals; makes suggestions to physicians to improve prescribing. Takes active role in training new staff. Is proactive in working toward department goals.
· Participates in at least one of the following: clinical pathway team; interdisciplinary team; hospital or department committee member; performance improvement team; JCAHO team; customer service team.
· Willingness to accept change and new responsibilities. Carries pager after hours and weekends.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
· Bachelors of Science from College of Pharmacy Accredited Required.
· BS AACP plus Pharmacy Degree, Pharm D, or Board Certification (Board of Pharmaceutical Specialties) preferred.
· 6 months experience plus 2 years of hospital experience in clinical role and/or equivalent education/training or ASHP Residency preferred.
· Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
